I agree to indemnity and hold harmless QC Hair/QC Professional for any loss, damage, claims or actions of any and every kind that I may incur as a result of applying and/or wearing QC Tape In Hair Extensions.

I agree that I will not hold QC HAIR/QC PROFESSIONAL  or its employees liable in anyway for any loss, damage and/or injury suffered by myself or my client as a consequence of my use of the QC Hair/QC Professional products or services.

I will indemnify and hold harmless QC Hair/QC Professional or its employees free from all loss damage, claims and actions of every kind that they incur or that are claimed or brought against them by any party as a consequence of my use of the QC Hair/QC Professional products and/or services.

I understand and agree that I will not give out any QC Hair/QC Professional forms/prices/release forms, Training Guide or any other QC Hair/QC Professional forms.
